Complaint Instructions

Insert the following into the Complaint Form:
 

  • Telephone:  Your home telephone number and daytime telephone number.
  • Title:  Select and mark your title.
  • Name:  Insert your name.
  • Address:  Insert your address.
  • Answer Questions 1 through 4 and add the specific information as shown.  Hand write
    your answers on the form.
  1. The electric transmission (New York State Electric & Gas) to Steuben Rural Electric
    Cooperative, Inc., my electric provider, in my area causes most of our outages.  Their
    reliability is terrible and needs to be improved.  Their outage caused an outage service on   __________  (date)  for __________ hours.
  2. Require New York State Electric and Gas to maintain their transmission system.
  3. I called 1-800-572-1131 during the last transmission outage on ________(date).
  4. I asked them to improve their reliability by maintaining the lines.

    (Insert the response you received from NYSE&G).
  • Date and sign the completed form.
  • Send the completed form to the address at the top of the form.
  • Advise Steuben Rural Electric Cooperative, Inc. office when you receive a response
    from the Public Service Commission.
